Safe Hospital Discharge (Virtual)

Join the Marie Jobling of CLC to talk about patient rights, the importance of having a healthcare advocate, and how to make sure you have a good discharge plan. The session will link to printed material for follow-up and participants will be invited to share experiences and tips for a safe transition from hospital to home.
